The time my impeller failed it was shortly after being installed at spring commissioning, unlike the usual vane coming off, the vanes
separated from the core. Looked like a perfectly good impeller
except the core was turning but not the vanes.
One of the tricks that I learned to insure the core of the impeller hasn't separated is to try to pull two vanes out from one another prior to installation. I've seen this happen even with new,in the bag, impellers.
