I've just recalled the occasional boat hook that has gone overboard during a mooring grab. I've seen it more than once. Grab pennant, pull to deck, try to drop boat hook to be able to use both hands, boat hook bounces handle on deck, then hook, then handle, then hook, while approaching the life lines. Plop.

Last I recall this, we were grabbing a mooring in 25kts of wind and, while the hook floats, there is no way we were getting in the water to get it. Hunted the shore the next day. Fauhgedaboutit
