I''m guessing he tried his "replacing a thru-hull with the boat still in the water" technique (lower down in this same catagory) and things went horribly awry.
What a sad way for a salty pirate to go, two feet underwater, wooden bung in one hand and the other accidentally water-proof glued to the hull.
