It is very likely the work of a scammer. I'd point out that the name, address and date of birth, combined with the mother's maiden name is generally enough for them to steal your identity in many cases. The mother's maiden name is a typical security question for many financial institutions, and there IS NO LEGITIMATE NEED FOR IT IN A JOB APPLICATION.
As for the typical security questions that financial institutions ask you to answer, I would recommend that you answer them with something other than the logical answer to the question. For instance, if you are asked "What was the name of your first pet?" Answer with the type or breed of dog it was instead of its name. The name of your pet is often easily discovered. As long as you answer the question the same way each time, the fact that the answer isn't "correct" for the question doesn't affect anything.
