I would say maintenance issues that could probably be avoided.

I nearly lost my last boat to a blown stuffing box hose that could/should have been avoided. I maintained my boat pretty well, but it's just one of those things that was always on next haul out's list to do.

Should have changed out that mooring pendant or cleaned the fuel tank or changed the fuel filters more regularly or shut the fuel off before I left the boat or shut the sea cocks or changed that rusty hose clamp or just stopped by the boat that one weekend but couldn't get to it...

It's a never ending battle and time flies.
