This list is from my empirical observations of 10 years liveaboard cruising mostly in the Caribbean.
Entering harbours at night.
Navigating through reefs at night.
Relying on a chart plotter when doing above. [Entering Luperon is a prime example.]
Mooring failure. Always dive on a mooring and check it.
But of course the biggest cause of loss/amage is almost certainly hurricanes. Andrew, Hugo Ivan Katrina.
