It would be a pain to put the flange back on to try now, but from what I remember, once the flange was pushed all the way in it was a tight fit.

Capt. Gary Randall, I'm with you that the flange is most likely just a softer metal that machined itself, as Faster said. Where I'm uncertain is whether this happened over weeks, years, hours? That would help determine to what extent this failure is a symptom of a larger problem.
