You probably won't find 100% agreement on the use of S/V. It's a personal choice and there is no requirement for recreational vessels to use it so there is no one definitive source to reference. It also differs depending on what country you are in.

Personally, I prefer to see S/V rather than s/v. I treat it like an acronym, which is usually (but not always, of course) capitalized.
