#1. is more likely where air gets trapped in the glass. Unless when you sanded it down a noticeable water came out.
#2 and #3 its possible that there just was enough epoxy to hold the gel coat?
Either or nothing to major to worry about - use the filler - epoxy, prime and paint over...Just make sure you follow the instructions to the T and keep the areas cleaned before and after each process.
