If tension is correct, neither too hard nor too loose, then you have alignment problems due to fatigue, broken pieces, loose screws that hold the guides, too much play on wheel sprocket, etc. If you're not proficient on mechanics, ask a good one to check your entire system. This should not be an usual thing ....
