Water Pump shaft on the move...

The raw water pump shaft on my Perkins 4-154 backed out of the pump body this weekend while underway and tore up the seal pretty badly. Replaced it with a spare but would like some input on potential causes. Belt too tight? Mars too far from the moon?

Thx
IUke
