Hi Mark,
I realize you are in the USA, but the Canadian Power Squadron (Canadian Power and Sail Squadrons) gives out a file for printing in all of their courses. It is the crib notes you wish to have. Perhaps e-mail them and see if you can get a copy? Perhaps the US Power Squadron may have something similar.

Note that you'll need a US version of the frequencies table as ours is slightly different.

I've seen placards & laminates for the VHF radio
And as far as stupid boat names, how about "On the Rocks"!? Think about it when trying to communicate with the USCG. There is a boat in the PNW that has that name.
