For day-sailing my list includes: ball cap (hat), deck shoes or sandals, swimsuit, Float Plan, Sailing Notes, Crew Sailing Safety Brief notes, Sunglasses and Backup sunglasses, first aid kit, small bag of emergency items, life-jackets as required, water proof (otter) boxes, portable GPS, Kleenex packs, Lunch/Snack Items, snorkel gear and fins (depending on location), swimming towel, bank cards, cell phone, cell phone case, duct tape, a few tools and lots of ice water.
I think that's enough for day sailing, but I haven't had to plan like that for sometime now. Our boat is loaded with those items and a lot more all the time, so I can just hop on and go.

As you note that when unprepared it can be miserable but what you really need is just about everything, including a well ordered mind to deal with the quotidian things that can go wrong. The list is too long to name here.
Ship happens.

There is no boat on the list. Hard to daysail without a boat. Very wet if you try.

Boonie hat, sun glasses, sun bloock, sweater, windbreaker, shorts, tee shirt, boat shoes and gloves.
That is what is in my back pack so when someone ask me to crew I can just grab it and go.
Actually I use this same stuff if I go fishing or motorcycle riding or to the races.
I hate to repack.

better to have it and not need it, than to need it and not have it. That should do it.
