As mentioned earlier there are good and bad hospitals everywhere including the states. Hospitals donít treat you doctors and nurses do but there again thereís good and bad. Big difficulty as a traveler or cruiser is to not have that local knowledge.
Hell, even when home I donít use my local hospital for some services but as a retired physician do have a leg up on whereís good for what. When in the Caribbean do get stories from cruisers telling you who to avoid. Still think main function of medivac insurance is to get you back someplace where youíre cued in to whose good.

Seems clear that this proclamation does not cover the short term visitor.........

Not sure what the 90 or 180 day cruising permit holder is required to do. I hate to say it, but I think one should show or purchase coverage at immigration, before being let past the turnstile. I'll gladly comply with the same, visiting other countries. I dare say, if a US citizen can afford to visit another country, they either have medical insurance or can afford it. There are those that choose not to have it. They are not the majority of the uninsured, by a long shot, but the rest of us are tired of paying for them, when they stiff their bill, because they spent their money on other fun stuff. It's just part of the death by a thousand cuts.
